LOAN APPLICATION PROCEDURE
The following information will be needed when applying for a loan:
- Borrower Information - Credit Review Form.
- Personal Financial Statement Form.
- A Property Information Form.
(Applications without a property under contract will not be processed. Your information will still be accessible in your account for future submissions.)
- Two most recent Bank, Credit Union or other Financial
Institution statements such as Savings accounts, Stock or IRA accounts.
Your name and date must appear on the statements.
- Copies of two most recent years Tax Returns. ( Form 1040 )
- Two most recent paystubs.
- An itemized list of needed repairs.
If you are Self-Employed you will need to submit copies of personal as well as
business bank statements along with copies of your tax returns for the two previous years.
Items 1, 2 & 3 are found online and can be completed and submitted from the website.

Items 4-7 are supporting documents which need to be submitted via fax for your loan to be considered.
Loans can not be processed until all supporting documents have been received.
Once all of the requested information has been reviewed and approved, we will send you an Appraisal Request Form and call
you to set up a meeting at our office. It is the borrower’s responsibility to meet the appraiser and provide access to the property.
Payment for the appraisal is to be provided at time of service.
Once the completed appraisal is received the Title and Survey are ordered through our attorney.
Forms will be provided to expedite this request. Please fax to the attorney as well as to us. When these last two items are received,
your closing can be scheduled and usually occurs within 5-7 days.
In some cases the appraisal and title/survey can be ordered simultaneously.
